Those are all correct, with Helmut being the Zemo with the unfortunate mask problem. Born was originally supposed to be the origin of the 616/Prime Punisher but ended up being the first appearance of the MAX version so Garth Ennis could ignore the sliding timescale.

Sidney and Loki's answers are all correct. The Brother Voodoo story, aside from the rape aspect, was also unusual in that Hembeck drew it in a serious rather than cartoony style.

Beyond! Beyonder was revealed to be the Stranger in the story, while Hank Pym was later revealed to be Criti Nol. Going from memory, I think the latter was hinted at but not actually stated

All those are correct. American Scream from the Vertigo series showed up in the Spectre series written by one of the writers in q.13, so it does appear to have been canon in that era
