    Those are correct. Storm was with the other Losers. The comic depicted them as the Earth-2 versions, though an argument can be made that they were normally on Earth-1. That leaves only the guy with the pendant and the dog

    Flower was who I was looking for. I just checked and some online sources claim he was Farmer Boy, but Crisis Index treats him as a new character, which makes sense because Sgt. Rock was still being published at the time and having a member killed by  Shadow Demons might have been hard to explain to non-superhero readers. I don't know if Farmer Boy appeared in Sgt Rock subsequently.

    Only one left, but you don't need to ask before guessing, just go ahead. And yes, that's the error. Who's Who clarified this.

    I have this Index, so I'm familiar with who's on the cover. And I may be misremembering this, but I'm pretty sure 10 Eyed Man was coloured properly in Crisis #12 and it may have been in one of the printings of the TPB where the mistake was made. Of course, he's hard to recognize without his costume and could easily be mistaken for a civilian. I know I did on my first reading of it.

    Correct. I remember reading Who's Who and being surprised to learn of his death in Crisis. Granted I hadn't read anything else with him in it but I thought I knew who all died in Crisis at least.
