In the entry for Captain America #319, it says that Letha's last appearance was in Marvel Two-In-One #96. However, she appeared in Thing #33 in between.

Just reading this now (I know...so behind on indexes and handbooks). And obviously this isn't really helpful since the trade's already out and whatnot...but...

Bernie is listed in the entry for Cap # 317 as appearing next in Cap # 327.

But then she's listed in the entry for Cap # 323. The Cap # 327 entry gets her last appearance right.
