Most of those are correct (though Young All-Stars is plural). I can't count 12, though, because the Ultra-Humanite was a recurring Golden Age villain, and while he used her body, Winters was deceased in her A-SS appearances. However, you're on the right track.
To elaborate on correct answers:
In the case of the Real American, he was used ironically, posing as a white supremacist. I think him being an android works better than Marvel having some white supremacist organization leaders be black (e.g. Sons of the Serpent). It undercuts the racism without the problem of the villain acting against his best interests.

Earth-Prime only appeared in the series in the JLA/JSA/A-SS crossover, after Per Degaton had already changed history.

Just noticed that question 13 got a bit garbled:

Terry Curtis, who Superman aided in Action Comics #21, was forced by the Ultra-Humanite to work for him as Cyclotron.

13. Which major villain, in a pre-Crisis sequence recounting events in a 1940s title, had to be drawn differently than the original story, and why?
Luthor. The Earth-2 Superman doesn't entirely jibe with the Golden age stories, which introduced elements linked to the Earth-1 version early on. So in the original story when Luthor used the Powerstone against Superman, he was bald, but in the All-Star Squadron version of the sequence, he had the red hair that Earth-1 Luthor lost in childhood.

14. During the Crisis, which backup storyline spanning multiple issues recounted an All-Star Comics story?
Lost in Space-Time. The original version had the JSA members sent to multiple planets in the solar system, all inhabited. Since what we know now makes that impossible, the retelling had them sent to other-dimensional worlds.

Those are correct. The 1970s Human Torch series has a different numbering than the 1940s series so I treated it as a different title.

The entire Hercules Unbound is collected in B&W, along with the entire Atomic Knights run, etc. in Showcase Presents: The Great Disaster. As with the final volume of Essential Tomb of Dracula, stories are presented chronologically in terms of when the story takes place rather than in publication order (though the Dracula stories are set in the past while the Great Disaster was DC's post-apocalyptic future).

I checked (the All-Star Companion vol. 2) and the storyline is listed there as "Shanghaied into Hyperspace!"

Those are all correct. Impact actually went by !mpact, so technically I fudged a little on the ABC, but I were to do a quiz on things beginning with !, it would be a very short quiz. The Marvel No-Prize Book has a bit of fun with the ice cream power.
